I. Instructions for medical treatment
Dear patient:
In order to make your medical treatment smooth and safe, please read the following instructions carefully:
1. The medical card is universal in our hospital and can be used in the outpatient department of our hospital.
2. All medical personnel who come to our hospital to see a doctor must be based on their real names (ID card names), and the medical cards shall not be borrowed or used in a masquerading way. If adverse consequences are caused, the patients shall bear their own responsibilities.
3. Be sure to keep your outpatient medical record properly, and do not alter, scratch, tear or damage it. The outpatient (emergency) medical records are an important basis for you to see a doctor and deal with medical related problems.
4. Paste all inspection reports (results) you accept in chronological order for proper preservation, and do not alter, damage or lose them.
5. When you see a doctor, you have the right to decide whether to accept the special examination (treatment) recommended or requested by your doctor. In case of special examination (treatment) or outpatient (urgent) operation, please sign the informed consent form in person or designate an agent. If the designated agent signs, please indicate the designated agent in writing at the corresponding position of the outpatient (urgent) medical record and authorize the entrustment. If you refuse to accept the item recommended by your doctor, please indicate your refusal in the medical record and informed consent form and sign it.
6. In the process of seeing a doctor, if you have any questions about the examination and treatment opinions taken by the doctor, please contact the doctor immediately. After taking the medicine, pay attention to the use of the medicine. If in doubt, please consult the pharmacist or doctor in time.
7. Please follow the doctor’s advice. If your condition changes, you should bring your medical card and medical records to the medical institution in time. Please do not use multiple medical records at the same time, to ensure the continuity of medical records.
II. Instructions for hospital discharge
Patients should bring the receipt of hospitalization advance payment when going through the discharge formalities, and go to the inpatient office to go through the discharge formalities. For discharge settlement of medical insurance patients, please bring the medical insurance application form and go to the inpatient office for settlement.
